Lakeridge Health Oshawa is seeking a Social Worker to support the Regional Nephrology & Diabetes Program.
The Renal Social
Worker will assist pre-dialysis patients in clinic and in-centre dialysis patients, with care planning and psychosocial support.
The role requires a Master’s degree in Social Work, registration with the OCSWSSW, and experience in nephrology social work, crisis intervention, and discharge planning. This is a temporary part time position in Oshawa.
