Accept service requests and conduct client assessments
Collaborate with community services for appropriate referrals
Monitor clients’ goals and provide regular updates
Advocate for individuals’ rights and choices
Assist with housing searches and bureaucratic navigation Requirements:
University Degree or College Diploma (Social Work preferred)
5+ years experience in developmental services
Knowledge of community support services
Solid communication and organizational skills
