Region. Key Responsibilities:
Offer comprehensive Nurse Practitioner services for palliative patients
Assess and provide consultative guidance for symptom management




Collaborate with Hospice staff on patient care
Maintain high standards in evaluating complex health situations
Travel to different Ontario Health atHome sites as required Requirements:
Currently registered Nurse Practitioner with the College of Nurses
Thorough understanding of community health services
Experience in palliative care education and management
Ontario Driver's License and vehicle access needed
Compliance with mandatory COVID-19 vaccination Leverage your Nurse Practitioner skills to significantly impact the lives of patients in the Erie St.
