Join ParaMed as a Clinical Planner, focusing on patient assessments and care plan development. Deliver quality personal support services in this full time position. In this role, you will be responsible for the onboarding process of current patients requiring various personal support services within your district.
As a Clinical Planner, you'll assess care needs, craft individual care plans, and ensure safety evaluations of their living settings. Moreover, you'll keep families informed about ParaMed policies and assist with accurate patient record management. Key Responsibilities:
Perform initial care needs assessments for incoming patients
Develop personalized care plans and safety evaluations
Advise on ongoing visit scheduling based on clinical needs
Inform families of ParaMed’s policies and care expectations
Keep accurate and current patient records at all times Requirements:
RPN/LPN registration with a minimum of two years’ experience
Understanding of healthcare laws and best practices
Excellent communication and organizational abilities
Proven competency in delivering patient-centered care
Vehicle access required for necessary travel Support patients in their care journey as a Clinical Planner with ParaMed.
