Position overview

Seeking a dynamic, organized and collaborative Support Coordinator to lead a Social Prescribing project.

This role will help strengthen partnerships within the healthcare system, expand services to underserved caregivers and support efforts to embed social prescribing into provincial care systems.

The successful candidate will help develop and implement tools to assess caregiver health outcomes, helping CNS better understand and measure caregivers’ physical and mental wellbeing. They will work with staff, caregivers, healthcare partners and community organizations to improve how caregiver needs are assessed and met.

Key responsibilities

Project Leadership

- Lead the planning, implementation and management of the Social Prescribing project to meet organizational goals and timelines.
- Collaborate with internal staff and external partners to achieve project milestones.
- Maintain project documentation, including work plans, reports and evaluation metrics.

Stakeholder Engagement

- Build and maintain relationships with Nova Scotia Health, healthcare professionals and community organizations.
- Serve as the primary contact for project partners and stakeholders.
- Organize workshops, presentations and meetings to promote the project and its benefits.

Outreach & Program Development

- Identify opportunities to expand services to underserved caregiver populations.




- Collaborate with communications staff on outreach materials and campaigns.
- Monitor and analyze referral trends to strengthen program reach and impact.

Evaluation & Reporting

- Collect and analyze data to measure project impact and caregiver outcomes.
- Integrate findings into CNS’s evaluation framework.
- Prepare reports and updates for funders, partners, and internal leadership.

Qualifications

- Post-secondary education in social sciences, public health, program evaluation, health research or a related field.
- Experience in program evaluation, research or assessment tool development.
- Strong analytical and research skills.
- Experience facilitating consultations with stakeholders or advisory groups.
- Excellent organizational and documentation skills.
- Proficient computer skills (email, internet, MS Office Suite, Google Drive).
- Strong written and verbal communication skills, including public speaking.
- Ability to work independently and collaboratively within a small team environment.
- High level of empathy and compassion; approachable and friendly personality.
- Ability to maintain confidentiality of caregivers and the organization.
- Knowledge of caregiving,



health systems or community-based services is considered an asset.

Caregivers Nova Scotia (CNS) is a province-wide non-profit organization dedicated to supporting family and friends providing unpaid care (also known as caregivers, unpaid caregivers).

1 in 3 Nova Scotians are or were at some point a caregiver. Through education, support, navigation and advocacy, CNS works to ensure caregivers are recognized, supported and included as essential partners in care.

Working conditions

- Monday-Friday, 8:30 a.m. to 4:30 p.m., with a one-hour unpaid lunch.
- Some evening and weekend work may be required.
- A reliable vehicle is required for travel to various locations; expenses will be reimbursed in accordance with Caregivers Nova Scotia's travel policy and the Government of Nova Scotia mileage rate.
