Provide impactful diabetes education and nutrition counseling at Anson General Hospital. As a Clinical Dietitian, you will advocate for patient-centered care and support the MICs Diabetes Program. This role encompasses both individual counseling and collaboration with an interdisciplinary team to improve patient outcomes. You will develop personalized care plans and educate patients on managing diabetes, ensuring adherence to qualified and program standards. Key Responsibilities:
Counsel patients on nutritional needs and diabetes management
Create customized care plans for diverse patient populations
Provide education to patients and their families
Collaborate effectively within a multi-disciplinary team
Maintain compliance with program and skilled requirements Requirements:
Bachelor's degree in Dietetics with completed internship
Certified Diabetes Educator or commitment to obtain
Active membership with the College of Dietitians of Ontario
Exceptional communication and documentation skills
