Review and assess incoming complex referrals
Conduct nursing assessments across multiple care domains
Coordinate schedules to ensure effective clinic operation
Anticipate and fulfill patient care requirements
Provide tailored education and counseling to patients Requirements:
Bachelor's Degree in Nursing
Unrestricted registration with the College of Nurses of Ontario
Three years’ clinical experience in geriatrics
Two years in a clinical coordinator role
