As a Community Inclusion Worker, you will support people in achieving their personal goals through various community involvement initiatives. This role requires solid communication and collaboration skills, along with a commitment to respecting each individual’s rights and choices. Your responsibilities will include coordinating activities, documenting personal health information, and implementing Individual Service Plans.
Key Responsibilities:
• Ensure individual rights are respected and advocated
• Facilitate community participation in activities and programs
• Empower individuals to be active community citizens
• Develop and document Individual Service Plans
• Record daily activities and personal health information
Requirements:
• Grade 12 education and Community Support Worker Diploma
• Minimum 2 years in community social services
• Emergency First Aid/CPR and Food Safe certification
• Valid BC Driver’s License and reliable vehicle
• Criminal record clearance for vulnerable adults
