As a vital member of the interdisciplinary care team, the Social Worker collaborates with staff to address resident and family needs. This role involves providing a range of planned interventions, financial/legal assistance, adjustment support, and consultation through comprehensive psychosocial assessments. You’ll utilize your skills to support residents and families effectively while fostering a comfortable environment.

Key Responsibilities:
• Conduct comprehensive psychosocial assessments for current residents
• Establish rapport through initial interviews with residents and families




• Provide counseling and support via therapeutic relationships
• Utilize various techniques to gather essential information
• Identify and document major life events with residents and families

Requirements:
• Registered Social Worker under Health Professions Act
• Bachelor of Social Work degree from an accredited institution
• Experience working with geriatric populations
• Strong skills in team collaboration
• Current Vulnerable Sector Check or willingness to obtain one
