Job Description:

10AM-10PM

Discusses facility core measures during handoff communication.

Develops individualized plans of care in collaboration with the patient, family, and healthcare providers, ensuring continuity and documentation.

Collaborates with interdisciplinary team members to solve admission and discharge problems.

Collects comprehensive patient medical history. Establishes and maintains positive relationships with patients and their families.

Provides family members with information about patient condition and plan of care.

Promotes a positive patient experience by demonstrating caring behaviors, maintaining clear communication, and collaborating with the care team to provide safe, timely,



and effective whole person care.

Follows hospital policies and procedures, making ethical decisions based on consent and confidentiality.

Collaborates with nursing staff and attending physicians in establishing patient admission and discharge processes.

Ensures compliance with healthcare admission policies and regulations. Maintains a safe and clean environment for patients.

Manages all aspects of patient admission and discharge processes.

Prepares, completes, and registers patient admissions.

Other duties as assigned.

Knowledge, Skills, and Abilities:

Education:

Associate's of Nursing [Required]

Bachelor's of Nursing [Preferred]

$33.99 - $63.22
