WORK SCHEDULE: 2-3 days/week
FLSA CLASSIFICATION: Part-Time, Non-Exempt
COMPENSATION: The compensation range for this position is $35,000–$55,000, with the opportunity to earn additional income through a variable compensation (commission-based) structure, offering higher earning potential based on performance.

Position Summary
Vitae Healthis seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ overall well-being and adjustment to medical and life changes.
Key Responsibilities
Conduct comprehensive psychosocial assessments for residents upon admission and as needed
Develop, implement, and monitor individualized treatment plans
Provide individual, group, and/or family therapy sessions
Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability
Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns
Collaborate withthe Vitae clinical team,
the patient’s attendingphysician,facilitynursing staff, social workers, and rehabilitation teams to coordinate care
Participate in care plan meetings and interdisciplinary team discussions
Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations
Provide crisis intervention and behavioral management strategies when needed
Educate staff and families on mental health conditions and effective communication strategies
Ensure compliance with HIPAA and ethical standards of practice
Master’sdegree in Psychology, Counseling, Social Work, or related field
Active and unrestricted license inrespective state(e.g., LCSW, LPC, LMFT, or Licensed Psychologist)
2years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings
Robust clinical assessment, diagnostic, and treatment planning skills
Knowledge of mental health conditions common in SNF populations (e.g., depression, anxiety, dementia, adjustment disorders)
Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling)
Ability to work collaboratively within an interdisciplinary team
Excellent communication and interpersonal skills with residents, families, and staff
Strong organizational, time management, and documentation skills
Familiarity with electronic health records (EHR) systems preferred
