Become an essential member of the Home Dialysis Program, providing comprehensive support for patients and families involved in dialysis. You will require two years of related nursing experience, along with strong leadership and organizational skills.

The position involves patient visits to their homes and maintaining clinical standards aligned with best practices.

Key Responsibilities:

- Perform and document detailed assessments of patients
- Educate patients and families on dialysis procedures
- Serve as the main contact for chronic kidney disease patients




- Work collaboratively with interdisciplinary health teams
- Prioritize urgent care needs and patient schedules

Requirements:

- Registered with the College of Nurses of Ontario
- Minimum two years of experience in dialysis nursing
- Leadership experience in nephrology preferred
- Current certification in BCLS/HCP is necessary
- Strong computer skills, including Microsoft Excel and Word
