As a Social Worker with the St. Michael’s Hospital Academic Family Health Team, you will collaborate closely with an interprofessional primary care team. Your role includes providing in-depth biopsychosocial assessments, direct care to new patients, and resource referrals. This position emphasizes educational support for patients and families while promoting anti-oppressive care practices.

Key Responsibilities:



• Facilitate community connections with interprofessional teams • Provide direct psychosocial care to new primary care patients • Conduct biopsychosocial assessments and treatment services • Collaborate in quality improvement and research initiatives • Teach and support staff and the academic community

Requirements: • Master’s Degree in Social Work • Licensed with OCSWSSW • Experience in primary care preferred • Current BLS certification or willingness to complete • Solid communication and interpersonal skills
