Excellent communication and interpersonal skills
Requires positive judgement, problem-solving solving and assessment abilities
The ability to work independently and cooperatively with home care coordinators

Key Responsibilities:
Conduct comprehensive health assessments and monitor patient conditions
Administer medications safely and accurately
Provide wound care and monitor healing progress
Manage chronic diseases and individualized care plans
Deliver palliative and end-of-life care
Assist patients with mobility and activities of daily living
Provide respiratory care,



including ventilator support and oxygen therapy
Perform catheter and ostomy care
Provide care for patients with G-Tubes and J-Tubes
Manage tracheostomies and IV therapy
Educate patients and their families on care routines, health management, and safety
Experience working with pediatric patients is considered an asset

Qualifications:
RN/RPN license valid in Ontario (Registered with CNO)
Vulnerable Sector Check completed or willing to complete
BLS Certification

Pay: $28.00-$37.00 per hour
