Contribute to community health as a Family Physician at Pinecrest-Queensway Community Health Centre. This part time role focuses on providing holistic primary care to diverse and underserved populations.As a Family Physician, you will collaborate within a multi-disciplinary team to offer comprehensive primary care services in the PQCHC area. The role includes diagnosing and managing a range of medical conditions while proactively addressing health maintenance and education. You will also conduct health assessments and coordinate referrals to ensure effective patient pathways.Key Responsibilities:
Deliver primary medical care following CPSO protocols
Treat and manage both acute and chronic health issues




Work with a team of health specialists for integrated care
Complete health assessments and develop comprehensive plans
Educate clients about chronic disease management and preventionRequirements:
Doctor of Medicine (MD) and CPSO licensure
Minimum of two years in family medicine
Experience with health equity for priority populations
Proficient with PS Suite EMR and Microsoft Office
