In this role, you will work with the Unify care team at Flemingdon Health Centre, providing clinical care and health promotion to the diverse populations of the community. You’ll perform assessments, coordinate healthcare services, and engage in ongoing health education and outreach. Evening and weekend shifts are required, aimed at enhancing community health initiatives.
Key Responsibilities:
• Conduct assessments and coordinate care strategies
• Provide telephone triage and medication administration
• Deliver health education and outreach support




• Participate in quality improvement and community projects
• Work collaboratively with interdisciplinary teams
Requirements:
• RN license with the College of Nurses of Ontario
• Bachelor of Science in Nursing (BScN)
• At least 1 year of experience in primary care
• Skills in assessment and counseling
• Willingness to work flexible hours
