As a Nurse Practitioner or Physician Assistant, you will establish long-term relationships with patients and support their health needs with evidence-based care. This role allows you to engage with patients directly and participate in telehealth visits, emphasizing continuity and quality of care. You'll work in an empowering environment, enhancing your skills while contributing to a population health model.

Key Responsibilities:
• Deliver in-home patient assessments and wellness visits
• Provide education on health management and preventive care




• Engage in virtual team discussions for integrated care
• Coordinate with specialists and document care effectively
• Perform point-of-care testing and prescribe medications

Requirements:
• Active NP or PA license and board certification
• Two years of direct patient care experience
• Current DEA certification and BLS certification
• Familiarity with EHR systems and HIPAA regulations
• Initiative and independence in decision-making
