The Winnipeg Regional Health Authority is seeking a Hospital Based Case Coordinator to assess referrals, plan and coordinate care to support secure discharge from hospital. You will collaborate with clients, families, and the interprofessional team to determine eligibility for Home Care supports and potential alternate care options.
Graduate of an approved RN program with Manitoba registration is required; three years of nursing experience is expected, with assets in hospital-based case
