The Nursing department has an opening for a Part-time RegisteredPractical Nurse at Emo Health Centre. The successful candidate must be able tomeet the normal requirements of the job and possess the followingqualifications:
QUALIFICATIONSNECESSARY:
CurrentCertificate of competence from the College of Nurses of Ontario as aRegistered Practical Nurse
Current CPR;
Demonstration of strong nursing practice, skilland judgement;
Possess excellent interpersonal skills includingdistinguished verbal communication abilities;
Demonstrated critical thinking, leadership andmentoring skills;
Exhibits the ability and competence to work withall computer systems and applications;
Exercises behaviours consistent with RiversideHealth Care’s Code of Conduct;
Willing to work flexible hours, including days,evenings, nights and weekends;

BASICRESPONSIBILITIES:
Provideand coordinate nursing care to assigned clients, patients, and residentsconsistent with current Standards of Professional Practice with theOntario College of Nurses.
Acceptshift rotation and assignments on other nursing units in accordance withstaffing needs.
Carry out all work assignments safely and beknowledgeable of and in compliance with relevant regulations, policies,and procedures
Familiar and in compliance with occupationalhealth & safety workers’ rights and responsibilities.
Suchother duties as may be delegated by the manager or supervisor.
SPECIFICDUTIES:
1 . Deliversdirect/indirect patient care according to the Standards of Practice of theCollege of Nurses of Ontario and in compliance with the philosophy,
goals andobjectives of the Department of Nursing, as follows:
a ) Assessment: Admits clients/patients, obtains a historyand initial and ongoing assessment of the nursing needs of assignedclients/patients.
b) Planning: Develops a plan of care which assignspriorities and prescribes nursing actions to achieve the desired outcomes foran individual and/or group of patients.
Developsa plan of care within 24 hours of admission considering:
i) ongoingassessment of the client’s/patient’s individual needs
ii) currentpriorities
iii) themedical/interdisciplinary care plan.
Assures the plan of care is feasible in terms of humanresources, time and materiel resources.
Revisesplan of care as dictated by the client’s / patient’s needs and medical planofcare.
c) Implementation: Co-ordinates and executes the physician’sorders according to the medical plan of care; coordinates the activities ofother health team members.
Performsnursing actions required for implementation of client/patient Care Plans.
Implementsa teaching plan based on the client’s/patient’s and families’ identified needs.
Providessupport to client/patient and family in understanding and coping withclient/patient illness.
Providescare in the designated area at a skill level which demonstrates the requiredknowledge and expertise.
Communicatesand consults with physicians and other health care professionals whenindicated.
Completes,systematically, the assigned duties conserving time, energy and supplies.
Assistclients/patients to achieve or maintain optimal independence.
Providefor the safety and well-being of the individual client/patient, including theenvironment.
d) Evaluation: Continuously evaluates the outcomes of thenursing care of clients/patients and modifies nursing measures and approachesaccordingly.
Evaluateswith the client/patient the results of nursing actions to increase or maintainself-care ability.
Reviseswith the client/patient, family, and relevant others the expected patientoutcome, priority and nursing actions as required.
Maintainsappropriate documentation and records of observations, nursing careadministered and client’s/patient’s response.
Ensurescontinuity of client/patient care through effective reporting at change ofshift.
2. Providessafe, competent, compassionate,
and ethical nursing care to ensure the personalsafety of clients/patients.
3. Usesappropriate assessment tools and techniques in consultation withclients/patients and the health care team to assess clients/patients regardingphysical, emotional, spiritual, cultural, cognitive, developmental,environmental, and social needs.
4. Engagesclient/patient using a collaborative approach, building rapport and trust,achieving a mutual understanding and agreement to identify actual and potentialclient/patient health care needs, strengths, capacities, and goals.
5. Establishesand maintains appropriate professional boundaries with clients/patients and thehealth care team, including the distinction between social interaction andtherapeutic relationships.
6. Participatesin necessary care conferences with Health Care team
7. Maintainsthe knowledge, skills, and attitudes to provide safe, efficient, effective carein all areas of practice by keeping abreast of changes in practice setting thataffect scope of practice.
8. Maintainsprofessional client/patient records related to client encounters and plans incompliance with the organizations policies and the standard applied by theirregulatory body.
9. Showsrespect for cultural needs, dignity, privacy, and confidentiality of clients
10. Mustbe able to organize time to ensure community and admitted clients’/patients’needs for assessments are met.
11. Communicateswith the health Care team with regards to functional ability within the home,perceived barriers in the home
12. Mustbe solution oriented to attain the goal of discharge home whenever possible
13. Participatesin departmental and related committees/meetings when required.
14. Participatesin educational activities and programs for clients/patients and staff.
15. Participatesin the orientation of new staff.
16. Reportsany untoward/unusual incidents to the Nurse Manager or designate, and recordappropriately.
17. Acceptsresponsibility and accountability for independent actions.
18. Attendsand participates in appropriate courses, conferences, and workshops to promoteprofessional growth and development.
19. Assistsin the evaluation of staff performance.
20. Demonstrates the ability and competence to work with all computer systems and applications.
21. Carryout all work assignments safely and be knowledgeable of and in compliance with relevant regulations, policies, and procedures.(i.e., Occupational Health and Safety Act).
22. Othersuch duties or responsibilities as may be delegated from time to time by theNurse Manager or delegate.
