Key Responsibilities:
• Collaborate with the interdisciplinary care team
• Provide patient-centered symptom management




• Coordinate care and create personalized care plans
• Participate in clinical huddles and case conferences
• Travel within the assigned region to provide care

Requirements:
• Registered with the College of Nurses of Ontario
• Minimum two years of nursing experience
• Experience in palliative care preferred
• Current CPR and First Aid certification
• Valid Ontario driver's license and vehicle
