In this role, you'll accept service requests from Developmental Services Ontario and conduct essential assessments for clients. Working closely with various community partners, you’ll identify needs and ensure that services are aligned with individual rights and self-determination. Your collaboration and advocacy will significantly impact the well-being of clients.
Key Responsibilities:
• Complete assessments to identify client goals
• Collaborate with community services for effective referrals
• Provide ongoing support and monitoring for clients
• Assist clients in navigating the judicial system
• Maintain accurate documentation and client records
Requirements:
• Bachelor of Social Work or related degree
• Minimum 5 years of relevant experience
• Proficient in conflict resolution and time management
• Excellent interpersonal and critical thinking skills
• Current Criminal Record Check required
