We are seeking an experienced Nurse Practitioner to join our team and deliver critical services to patients facing palliative needs. This role includes diagnosing, treating, and managing chronic disease symptoms while performing assessments and providing expertise throughout the patient's care journey. Flexibility to travel among Erie St. Clair Region sites is essential for collaborative care.

Key Responsibilities:
• Provide tailored Nurse Practitioner services to patients in palliative care
• Engage in assessment and consultative efforts for symptom resolution




• Work closely with Hospice and healthcare teams for integrated care
• Adhere to Ministry guidelines and organizational standards
• Monitor and evaluate patient progress effectively

Requirements:
• Current registration as a Nurse Practitioner with the College of Nurses
• Proficient knowledge of community healthcare dynamics
• Specialized palliative training, including CAPCE
• Must possess a valid Ontario Driver’s License
• Adhere to COVID-19 vaccination policy
