The role is centered on enhancing personal connections and guiding families through significant changes. By facilitating meaningful conversations, you can empower individuals to envision and navigate their futures.
Key Responsibilities:
• Facilitate networks and future-planning conversations
• Build deep relationships with families and individuals
• Support families through transitions and crucial life changes
• Develop and sustain personal support systems
• Connect with community resources and partners
Requirements:
• Extensive experience in family support or disability services
• Solid relationship-building and facilitation expertise
• Knowledge of person-centered practices and approaches
• Maturity and respect for professional boundaries
• Excellent organizational and communication abilities
