: Reporting to the Primary Health Care (PHC) Manager, the Primary Health Care (PHC) Nurse is an integral member of the PHC team, providing and coordinating initial, continuing, and comprehensive nursing services to patients in the PHC site. The PHC Nurse promotes quality health care outcomes working within established standards to ensure patients/clients receive a high standard of primary health care. The PHC Nurse works alongside physicians, clinic staff, and other health care professionals to promote quality health care outcomes, while working with established standards to ensure patients/clients receive a high standard of primary health care.PHC Nurse initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site.

The PHC Nurse works relatively independently and is expected to make decisions based on competent assessment knowledge and skills.

Human Resources Exemption: No

Experience

- Minimum one year of recent nursing experience in a clinic-based setting required
- Experience in teaching adults, facilitation skills, and leading group activities

Education

- Bachelor of Science in Nursing (BScN)

Other Education and Training

- Nursing diploma is acceptable




- Basic Life Support Certificate (BLS)

- Knowledge of the impact of social/economic/cultural issues on chronic disease
- Ability to work effectively with individuals who are First Nations or Metis
- Knowledge in chronic disease prevention and management
- Makes independent clinical judgment based on evidence based best practice
- Competent critical thinking/judgement/autonomous decision making capabilities
- Knowledge in LEAN management system
- Knowledge of group dynamics and facilitation skills.
- Ability to work under pressure and establish priorities
- Ability to work independently and as a member of a multi-disciplinary team
- Demonstrated understanding of the prevention and management of chronic diseases
- Knowledge and application of quality improvement principles, tools, and outcomes
- Proven conflict resolution skills
- Demonstrates ability to accept and implement change
- Demonstrates effective verbal and written communication skills.
- Ability to use health promotion and prevention strategies for program planning
- Demonstrates empathy, trust and respect in the provision of care

Other Information





- Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
- Experience in case management in health care setting is an asset
- Ability to travel when required
- May be eligible for Recruitment and/or Retention Incentives
- Experience in a community-based setting (Public Health, Home Care) is an asset.
