Centre (IOC) at AHS is seeking a Patient Flow & System Coordination Specialist. This role focuses on facilitating patient transfers, repatriation, and complex care transitions across multiple acute care sites. Collaborating with various stakeholders like physicians and community partners, you'll address barriers and ensure timely patient access to care. Key Responsibilities:

- Coordinate patient flow and transfers between facilities
- Monitor and manage capacity pressures in real-time




- Collaborate with multidisciplinary teams and community partners
- Support optimization of clinical information systems
- Facilitate evidence-informed decision-making and operational problem-solving Requirements:
