Reporting to the Clinical Manager, you will assess patient needs, implement care plans, and ensure follow-up services. You will use your nursing skills to work from a health equity framework to boost health access and outcomes. Key Responsibilities:

- Conduct assessments and provide nursing interventions
- Collaborate on individualized nursing care plans
- Perform IV therapy and clinical procedures
- Advocate for holistic client care and resource access
- Document care effectively using Telus PS Suites Requirements:
- Valid R.P.N.

- At least 2 years of nursing experience in a related setting
- Experience with medication administration and safety protocols
- Proficiency in using Electronic Medical Records
