You will oversee clinic visits and perform dialysis rounds, ensuring holistic patient assessments and treatment plans. Collaborate with an extensive network of over 1,600 providers while contributing to quality improvement initiatives. Key Responsibilities:

- Provide comprehensive nephrology care across clinical settings
- Assess patients and develop effective care strategies




- Conduct dialysis rounds and collaborate with healthcare teams
- Document all patient interactions in Epic software
- Assist with chronic disease management and patient education Requirements:
- Completed Master’s in Physician Assistant or Nurse Practitioner
- Illinois state licensure or eligibility
- Experience in nephrology (1-2 years preferred)
