Remember When HealthCare is expanding our community care services and seeking Licensed Practical Nurses (LPNs) to join our growing team. As a Community Care Manager, you will provide high-quality nursing care to clients in their homes, complete recent client assessments, and support care continuity across our communities.

Key Responsibilities

- Conduct in-home nursing assessments for new and existing clients
- Provide education and guidance to clients, families, and caregivers
- Deliver nursing care including medication administration, wound and catheter care, and health monitoring
- Perform initial and follow-up health assessments
- Maintain accurate records and ensure timely communication with the office team




- Collaborate with family members and interdisciplinary healthcare professionals to ensure comprehensive care
- Provide information about services to clients and families
- Support caregiver training and conduct home or facility-based audits as required

Qualifications

- Licensed Practical Nurse (LPN) with active registration in Nova Scotia
- Valid driver’s license and reliable vehicle
- Current CPR/BLS certification
- Clear Criminal Record and Vulnerable Sector Checks

Schedule

- Flexible scheduling based on your availability and client needs
- Candidates should have at least 1–2 days of availability per week
