Become a key player as a Community Health Worker at LAMP Community Health Centre. Focus on client intake and health promotion while serving diverse populations in Etobicoke. In this position, you'll report to the Health Promotion & Community Support Manager.

Your role includes conducting assessments, facilitating community programs, and collaborating with interdisciplinary teams to enhance health service accessibility. You will work through an anti-oppression lens to promote health equity and improve outcomes for priority populations. Key Responsibilities:

- Conduct comprehensive client assessments
- Facilitate group health programs and lifestyle workshops
- Build partnerships for community outreach
- Support program communication and awareness
- Manage data collection for program evaluation Requirements:
- Post-secondary education in a relevant field
- 3+ years of experience in outreach or community health
- Robust communication and facilitation skills
- Knowledge of health equity frameworks
