Job Summary

Join our dedicated healthcare team as a Licensed Practical Nurse (LPN), Registered Nurse (RN), Registered Psychiatric Nurse (RPN), or Personal Support Worker (PSW). In this vital role, you will provide compassionate, patient-centered care across diverse settings, including nursing homes, hospitals, assisted living facilities, and home care environments. Your expertise will help improve quality of life for patients of all ages, from pediatrics to seniors, ensuring they receive safe, effective, and respectful care.

Responsibilities

- Deliver comprehensive patient care by assessing needs, administering medications, and monitoring vital signs with precision and compassion.
- Assist with daily activities such as bathing, dressing, mobility support, and nutritional needs to promote independence and dignity.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans tailored to each patient's unique requirements.
- Maintain accurate documentation of patient progress, medication administration, and clinical observations using electronic health record systems like Epic or Cerner.
- Support specialized care areas including dementia care, hospice care, pain management, pediatrics, and senior care to ensure holistic treatment.




- Conduct laboratory procedures such as phlebotomy and handle specimen collection with strict adherence to safety protocols.
- Educate patients and families about health conditions, medication regimens, and self-care strategies to foster understanding and engagement.

Requirements

- Valid licensure as an LPN, RN, RPN or certification as a PSW; relevant state or provincial credentials required.
- Prior experience in nursing homes, hospitals, acute care settings or home care environments is highly preferred.
- Demonstrated knowledge of medical terminology, medication administration protocols, and patient safety standards.
- Experience working with diverse populations including children, seniors, or hospice patients is advantageous.
- Familiarity with electronic health records systems such as Epic or Cerner; medical office experience is a plus.
- Strong communication skills coupled with the ability to work effectively within multidisciplinary teams.
- Ability to handle laboratory procedures like phlebotomy and manage case documentation efficiently.
