This is a non-prescribing, field-based role focused on preventive care, patient education, identifying care gaps, and collaborating with primary care providers to improve health outcomes. Conduct comprehensive in-home health assessments Review medical history and medications Perform physical exams, screenings, and point-of-care testing Identify and document diagnoses for care planning and treatment management Communicate findings to primary care providers Educate members on disease management, medications, and preventive care Address social determinants of health and connect members with community resources Recognize and respond appropriately to urgent or emergent situations NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care Ability to work in a variety of home environments and travel extensively 1+ years of clinical experience in family practice, geriatrics, primary care,

Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
