The North Simcoe Family Health Team (NSFHT) is seeking a full time, permanent position.

Job Title: Occupational Therapist – Seniors Program

Salary range: $38.48 - $40.83 per hour

Reports to: Clinical Director The NSFHT is a multi-disciplinary team located in Midland Ontario. We provide primary care, education, and counselling services to patients in the North Simcoe Community. We offer a friendly, team-oriented, and professional environment.

Position Summary: The North Simcoe Family Health Team (NSFHT) is seeking a compassionate and skilled Occupational Therapist (OT) to join our Seniors Program. The OT will provide client-centered assessment and intervention to support older adults in maintaining independence, safety, and quality of life. Working within an interdisciplinary primary care team, the OT will address functional, cognitive, mobility, and environmental needs of seniors and their caregivers.

Responsibilities and Job Expectations

- Conduct comprehensive occupational therapy assessments for seniors, including functional abilities, cognition, mobility, falls risk and home safety.
- Develop and implement individualized goal-oriented care plans focused on independence, aging in place, and quality of life.
- Provide interventions related to activities of daily living (ADLs), energy conservation, adaptive equipment, and environmental modifications.
- Deliver falls prevention strategies, mobility recommendations, and safety education.
- Support seniors with chronic disease management, cognitive changes, frailty, and functional decline.
- Provide education and support to caregivers and family members.
- Collaborate closely with physicians, nurse practitioners, nurses, social workers, clinical coordinator, and other allied health professionals.
- Participate in interdisciplinary case conferences and care planning.




- Coordinate referrals to community services and supports (geriatric services, assistive devices programs).
- Maintain accurate and timely clinical documentation in electronic medical records (Accuro).

QUALIFICATIONS AND SKILLS

Education and Professional Memberships

- Master of Science in Occupational Therapy
- Current registration in good standing with the College of Occupational Therapists of Ontario (COTO)
- Experience working with seniors in primary care, community health, rehabilitation, or long-term care settings.
- Strong knowledge of geriatric care, aging-related conditions, and functional assessment.
- Experience with falls prevention, home safety assessments, and assistive devices.
- Excellent communication, organizational, and teamwork skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
- BCLS.
- Assistive Device Program (ADP) authorizer status may be preferred for prescribing mobility and ADP equipment.

Work Schedule: This is a full-time, permanent position with a standard 37.5-hour work week, Monday to Friday.
