Salary Range: $41.15 - $58.98 /hour

This program provides $25,000 in grant funding to eligible RPNs/RNs/ Nurse Practitioners in exchange for a two-year full-time commitment. The Registered Nurse with the Geriatric Ambulatory Access Team will be part of an exciting team that supports patients/family caregivers in accessing Community Geriatric Services through assessment and triage by telephone assessment in an office environment.

Essential Qualifications

- Valid registration with the College of Nurses of Ontario
- Valid CPR certificate
- Previous experience with assessments in an inpatient or community-based geriatric population
- Excellent problem-solving skills and ability to prioritize in situations of varying degrees of complexity
- Ability to function as an independent practitioner and a highly effective team member
- Knowledge of how to mobilize community health resources and systems
- Ability to synthesis clinical information succinctly and accurately both written and verbally




- Exceptional verbal and written communication skills with a variety of community and organizational partners
- Demonstrated application of case management skills in complex clinical situations
- Superior interpersonal and relationship building skills
- Ability to be flexible in the scheduling of work hours

- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA

Preferred Qualifications

- BScN
- Experience with Cerner and Clinical Connect
- Knowledge of registration and scheduling

Immunization Requirements

- Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
- Provide documentation of the Tuberculosis skin testing
