The North Simcoe Family Health Team (NSFHT) is seeking a full time, 12-month contract position.
Job Title: Registered Nurse – Seniors Program
Salary range: $39.51 - $41.93 per hour
Reports to: Clinical Director The NSFHT is a multi-disciplinary team located in Midland Ontario. We provide primary care, education, and counselling services to patients in the North Simcoe Community. We offer a friendly, collaborative, and qualified environment.
Position Summary
The North Simcoe Family Health Team (NSFHT) is seeking a skilled and compassionate Registered Nurse (RN) to join our Seniors Program. The RN will provide comprehensive nursing care to older adults, supporting health maintenance, chronic disease management, and aging in place. Working within an interdisciplinary primary care team, the RN will play a key role in care coordination, clinical assessment, and patient and caregiver education.
Responsibilities and Job Expectations
- Conduct comprehensive nursing assessments for seniors, including physical health, functional status, cognition, frailty and falls risk.
- Monitor and refer to FHT programs for management of chronic conditions (e.g., diabetes, hypertension, COPD).
- Support medication management, reconciliation, and adherence in collaboration with prescribers and pharmacists.
- Provide health promotion, disease prevention, and self-management education to seniors and caregivers.
- Participate in care planning for complex and medically fragile seniors.
- Identify early signs of health decline and coordinate timely interventions.
- Collaborate with physicians, occupational therapists, nurse practitioners, social workers, clinical coordinator, and other allied health professionals.
- Participate in interdisciplinary case conference and Seniors Program Planning.
- Coordinate referrals to community supports and services (geriatric services).
- Maintain accurate, timely documentation in electronic medical records (Accuro).
- Contribute to quality improvement initiatives and population health activities.

QUALIFICATIONS AND SKILLS
Education and Professional Memberships
- Baccalaureate of Science in Nursing.
- Registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse.
- Experience working with seniors in primary care, community health, geriatrics, or chronic disease management.
- Effective communication and interpersonal skills.
- Knowledge of geriatric syndromes, frailty, polypharmacy, and aging-related conditions.
- Excellent communication skills.
- Ability to work independently and as part of a team.
- BCLS.
Work Schedule: This is a full-time, 12-month contract position with a standard 37.5-hour work week, Monday to Friday.
