PCH is seeking a Registered Social Worker for Wellbrook Place East. This role requires collaboration with a diverse team to assess and fulfill the psychosocial needs of residents. Key responsibilities include planning care services, facilitating resident transitions, and supporting advanced care planning discussions.
Key Responsibilities:
• Collaborate with healthcare team to assess resident needs
• Coordinate resident moves and family introductions
• Develop and implement individual social work services
• Advocate for resident rights and independence




• Organize care conferences and family support groups
Requirements:
• Registered Social Service Worker as per provincial standards
• Minimum three years' experience in healthcare or social services
• Preference for experience in Gerontology
• Solid communication and organizational skills
• Ability to work collaboratively in teams
