Key Responsibilities:
• Implement AgeCare policies and procedures alongside Registered Nurse
• Liaise with physicians and external healthcare partners
• Notify physician of significant resident status changes
• Direct Personal Support Workers on care delivery
• Participate in shift reports to inform team of resident updates
Requirements:
• Knowledge in geriatric and rehabilitation nursing
• Current Certificate of Competence from College of Nurses of Ontario
• Familiarity with healthcare regulations and acts
• Strong teamwork and communication skills
• Experience in long-term care preferred
