Sanctum 1.5 is a transitional care home for pregnant women who are at high risk of contracting HIV, are HIV positive or in active substance use. We offer a supportive home for women and facilitate mother-infant bonding and reduce child apprehensions at birth.
The home support worker provides care and support to clients. Clients may also be affected by mental illness, addiction, poverty, homeless or other psycho-social factors. The home support worker will embody a patient centered harm reduction care model fostering an environment of acceptance and understanding for residents.
Day Shifts - 07:30-16:00
Evening Shifts - 15:30-00:00
Night Shifts - 23:30-08:00
Responsibilities
- Personal care & hygiene
- Assist with and support clients with medication adherence.
- Build rapport with residents and provide therapeutic counselling
- Follow medical instructions carefully and consistently.
- Assist with meal preparation and planning.
- Household duties such as cleaning and maintenance
- Manage any unanticipated events or unstable situations.
Qualifications
- Harm Reduction & Trauma and Violence Informed Care.
- Mental health & Addictions
- Cultural sensitivity
- Conflict resolution and interpersonal skills
- Robust Decision-making in varying situations
- Degree/diploma or certificate in social work, care aid, addictions, or related field
- Must retain active CPR and First Aid certification.
- Must retain Food Safety Certificate
- Must retain TLR Certificate
- Naloxone Training is an asset
