You will work closely with healthcare partners and community agencies to enhance access to health services for diverse populations facing barriers. Key Responsibilities:
Manage and supervise primary care intake processes
Prioritize client intakes based on urgency
Ensure compliance with documentation standards
Collaborate with clinicians and administrative teams
Track referral follow-ups for service initiation Requirements:
Valid Nurse registration in Ontario
Two years' experience in primary care or community settings
Knowledge of social determinants impacting health
Proven communication skills in English, a second language is an asset
