About the role: PrimePath Community Care is looking for a warm, patient, and reliable Support Worker to provide 1:1 in-home and community-based support to a young adult with a developmental disability. Responsibilities include personal care assistance, behavioural support using a structured, positive-reinforcement approach, community outings (music, swimming, walks), and close communication with our clinical team and the client's family.
What You'll Do
- Provide compassionate 1:1 support with daily routines and personal care
- Follow an individualized behaviour support plan with consistency
- Support participation in community activities and leisure programs
- Use visual schedules, positive reinforcement, and calm de-escalation techniques
- Accurately document daily logs, incidents, and medication administration
- Communicate professionally with family and the PPC care team
What We're Looking For
- Experience supporting individuals with developmental or intellectual disabilities
- Current Vulnerable Sector Check and Standard First Aid/CPR
- Patient, calm, and consistent communication style
- Reliable, punctual, and comfortable with a structured care plan
- Valid driver's license an asset, not required
