You will be responsible for health assessments, diagnosing, and developing holistic treatment plans. Your collaboration with an interdisciplinary team will create a seamless care experience for patients requiring comprehensive support. Key Responsibilities:
Deliver primary care services to unattached patients
Create and implement patient-centered care plans
Collaborate with healthcare and community specialists
Participate in health promotion activities
Monitor patient progress and adjust care plans Requirements:
Must hold a valid NP registration in Ontario
At least 2 years of independent practice experience
Proficient in electronic medical records
Excellent clinical and communication skills
