Experience with substance use concerns and intimate partner violence is essential to provide tailored support for transitioning patients. Key Responsibilities:
Conduct clinical social work assessments for patients
Develop individualized care plans including referrals
Provide psychosocial support during transitions

Requirements:
Master of Social Work (MSW) degree
Minimum 2 years experience with mental health
Registration with Ontario College of Social Workers
