This full time temporary role at Toronto Rehab - University Centre seeks a creative and cooperative social worker to support patients with movement disorders.

You will lead psychosocial assessments, contribute to comprehensive treatment plans, and engage with a multidisciplinary team to enhance patient experiences and outcomes. Key Responsibilities:
Deliver psychosocial assessments for patients with neurological disorders
Formulate comprehensive treatment strategies within a team
Actively participate in multi-disciplinary care coordination




Follow up with patients via telehealth and in-person visits

��� Facilitate community connections to support patient needs Requirements:
Master’s Degree in Social Work is needed
Registration with Ontario College of Social Workers required
At least four years in clinical practice
Skills in cognitive behavioral therapy are advantageous
