Vancouver Coastal Health is seeking an Indigenous Intake and Transition Support Nurse to join the Indigenous Mental Health team. This role provides health system navigation for Indigenous patients across acute and community health contexts, enabling smooth transitions and coordinated care.
You will work with care teams to develop procedures and educate staff, prioritizing culturally protected practices, equity and reconciliation. Preference will be given to Indigenous applicants.
