LOFT Community Services is seeking a Case Manager for the M-DOT Hotel Team in Toronto. You will provide direct in office and community-based case management to individuals experiencing homelessness, with a focus on mental health, health, and addictions issues.
You will work with a multidisciplinary team, including nurses and physicians, to assess, plan, treat, and connect clients to housing, ID, and social supports. A valid Ontario driver’s license and access to a vehicle are required.
