Title: Licensed Practical Nurse, Long Term Care
Location: Kelowna,BC
Hours:
Full time night line, 630pm-630am, rotating between 3 days on, 2 days off, 3 days on, 3 days off
Part time day line, 7am-3pm, rotating between 3 days on, 2 days off, 3 days on, 3 days off
Responsibilities: Work as a member of the inter-skilled team as described by our Professional Practice Model applying the person-centered approach to care.
Complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required.
Implement and evaluate the effectiveness of the care/service plans and make changes as required.
Assist with scheduling of resident's care conferences and participate in conferences where applicable.




Work closely with the Director of Care to facilitate resident's move in, move out and transfers.
Provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Resident Care standards, policies, and procedures.

Qualifications: Practical Nurse Diploma
Active BCCNM License Registration
Prior Canadian experiencein senior care or long-term care
Current CPR training certification
Current First Aid certification
Completion of a current Criminal Record and Vulnerable Sector Check (or willingness to obtain)
