As a vital member of Sobeys' national pharmacy team, you will deliver comprehensive clinical services tailored to continuing care obligations. Your responsibilities will include initiating care plans, medication reviews, and educating nursing staff. Participate actively in a cooperative, patient-focused environment to ensure the health and well-being of our communities.
Key Responsibilities:
• Provide patient-centered clinical services and care plans
• Conduct comprehensive medication reviews and assessments
• Educate nursing staff on medication-related topics
• Collaborate with the healthcare team for optimal patient outcomes
• Prioritize prescription safety and adhere to regulatory requirements
Requirements:
• Bachelor of Science in Pharmacy from a recognized institution
• Licensed Pharmacist in good standing
• Injection and First Aid Certification required
• Preferred experience with Kroll software
• Additional certifications like Geriatric Pharmacist advantageous
