Working at PACE, a nonprofit dedicated to helping over 350 individuals live independently, you will facilitate effective service planning and enhance client well-being. Your role will bridge gaps between clients, healthcare providers, and community services, ultimately improving access to essential resources. Make a difference with your expertise in community support.
Key Responsibilities:
• Empower clients through life skills coaching
• Develop partnerships with community organizations
• Coordinate client transitions across care environments
• Share educational resources for informed client decisions
• Maintain program quality and accurate client records
Requirements:
• Post-secondary education in a relevant area
• 3 to 5 years experience in community support
• Proficiency in case management and system navigation
• Awareness of AODA and health service standards
• Essential conflict resolution and empathy skills
