This full time, unionized role requires availability for evenings and weekends. You will assist clients in a transitional housing program, maximizing their capacity for sustainable living through support and community connections. The ideal candidate possesses a post-secondary degree in Human Services and demonstrates empathy, accountability, and responsiveness.

Key Responsibilities:
• Work with families on individualized housing plans
• Respond to community referrals and calls
• Place families in permanent housing units




• Assess needs using acuity tools for service delivery
• Maintain effective communication with housing providers

Requirements:
• Degree/diploma in Human Services or related field
• Registration with Ontario College of Social Workers
• Experience with mental health and addictions issues
• Flexibility for evening and weekend shifts
• Valid Driver’s license and access to a vehicle
