As a Wellness Coordinator (RPN), you’ll play a crucial role by coordinating care services and engaging with Community Members, families, and healthcare professionals. Responsibilities include wellness assessments, developing care plans, and overseeing medication administration. Key Responsibilities:
• Assist in planning and implementing Community Member wellness services
• Conduct assessments for new and existing residents




• Develop and evaluate individualized care plans
• Collaborate with healthcare professionals for continuity of care
• Maintain accurate health records and documentation

Requirements:
• Current RPN license and 3 years’ nursing experience
• Experience with older adults and assessments
• Strong communication and organizational skills
• Familiar with Microsoft Office Suite
• Valid CPR/First Aid Certification needed
