Our non-profit organization is looking for a committed RN/RPN to fulfill essential responsibilities in client assessments, case management, and clinical support. Key Responsibilities:
• Assess client needs and deliver case management support
• Serve as a clinical consultant for complex health cases
• Submit Initial and Interim Client Assessment Reports




• Attend meetings for continuous quality program improvement
• Offer advice to the Infection Prevention and Control Committee

Requirements:
• Nursing or Practical Nursing degree/diploma required
• At least 3 years of community healthcare experience
• Active member of the College of Nurses of Ontario
• Current First Aid & CPR certification
• Robust communication and organizational skills
